HS4U

Please use the HS4U database for Service Learning and/or Academic Internship site placement. Below are steps that will help you navigate HS4U.  To be prepared, please make sure you have secured faculty approval for your chosen site, have your student ID number, hours of service required for the class, your site supervisor's name and email address, and your learning and service outcomes for your class and placement. 

 

Step 1: Go to app.calstates4.com/humboldt, then click Student/Faculty login.
Step 2: You will automatically be redirected to your myhumboldt.edu log in. Enter your humboldt login information.
Step 3: Look for your course/class number and click place.
Step 4: You will be redirected to this page. Click search by site.
Step 5: Look for the organization/site that your faculty has approved for your placement.
Step 6: Once you have found your site, click on Select this site.

Step 7: You will be directed here. Enter the number of hours and complete the form. Talk with your faculty as you may be asked to do the following steps:

  1. Estimated number of service hours
  2. Site Staff Supervisor - Leave this area blank

Step 8: Fill out the Signup Forms.  There are two required forms.

The "Student Learning Plan, Participation Guidelines & AV Release" and the "Release of Liability."  

Once you complete these forms you will need to click the button to finish placement. 

Step 9: Once you have completed your forms, go to the homepage. Then click "View my placements."
Step 10: View your submissions, then download and print the Humboldt Student Time Log.

Step 11: 

Your instructor may require your "Student Learning Plan, Participation Guidelines & A/V Release" to be printed and signed by your site supervisor.  

Click on the view submission button for this document and it will open a printable PDF with places for partner signatures. 

Have your site supervisor place their signature and printed name next to your own on the document.  Then hand them into your instuctor.

Your Time Log should be completed by the end of your service, signed, and then handed in to your professor.
